In many cases, a thorough evaluation reveals that the neck, the upper back, and the surrounding musculature are playing a much larger role in driving symptoms than the person realized. Identifying and addressing those contributing factors directly can produce meaningful and lasting improvement in both pain and function.

Below is an overview of the most common head and neck conditions we treat in Winter Park and the greater Orlando area.

Neck Pain and Stiffness

Neck pain and stiffness are extremely common in both athletes and active adults and can develop from a wide range of causes including acute injury, repetitive strain, postural habits, and degenerative changes that accumulate over time. Cervical Strain, Sprain, and Muscle Spasm

Cervical strains and sprains involve injury to the muscles, tendons, or ligaments of the neck and are among the most common causes of acute neck pain. They can develop from a sudden movement or impact, from sleeping in an awkward position, or from overexertion during exercise or sport. Muscle spasm, which often accompanies cervical strain, involves involuntary contraction of the neck muscles that can feel severe and limiting even when the underlying tissue injury is relatively minor.

Despite how uncomfortable acute cervical strain and spasm can feel, these conditions generally respond very well to early, appropriately managed movement combined with hands-on Physical Therapy treatment. Whiplash-Associated Disorders

Whiplash-associated disorders develop following a rapid acceleration-deceleration injury to the neck, most commonly from a motor vehicle accident but also from sports impacts and falls. The sudden forceful movement can strain or sprain the soft tissue structures of the cervical spine, and symptoms can include neck pain, stiffness, headaches, dizziness, and in some cases arm pain or cognitive symptoms.

Whiplash injuries exist on a spectrum of severity, and recovery timelines vary significantly depending on the extent of the injury, the presence of neurological symptoms, and how quickly appropriate management begins. Early Physical Therapy has been shown to produce better outcomes than prolonged rest and collar immobilization, which were historically recommended but are now understood to delay recovery.

Treatment focuses on restoring cervical range of motion, reducing pain and muscle guarding, addressing any contributing upper back and shoulder stiffness, and progressively rebuilding strength and movement confidence in the neck and surrounding musculature.

Disc-Related Neck Pain and Cervical Radiculopathy

Disc-related neck pain occurs when one of the intervertebral discs of the cervical spine becomes irritated, bulges, or herniates in a way that causes local neck pain or pressure on a nearby nerve root. Degenerative Disc Disease of the Cervical Spine

Degenerative disc disease of the cervical spine is one of those diagnoses that sounds far more alarming than it typically needs to be. The term degenerative implies a progressive and irreversible deterioration, and for many people receiving this diagnosis the immediate assumption is that their condition will only get worse over time.

The reality is more nuanced. Degenerative changes in the cervical spine are extremely common and are found on imaging in a large percentage of middle-aged and older adults with no neck pain at all. The presence of degenerative changes on an MRI does not determine whether those changes are causing your symptoms, and it certainly does not determine your prognosis.

What Physical Therapy addresses is not the structural change itself but the pain and functional limitations associated with it. Improving cervical and thoracic mobility, building the strength and endurance of the muscles that support the neck, and optimizing movement patterns to reduce unnecessary load on the degenerative segments can produce meaningful and lasting improvement in pain and function for the vast majority of people with this diagnosis.

Cervicogenic Headaches

Cervicogenic headaches are headaches that originate from structures in the cervical spine, most commonly the upper cervical joints, muscles, and soft tissues. They typically present as a one-sided headache that begins at the base of the skull and radiates forward toward the eye or forehead, and are often accompanied by neck pain and stiffness and restricted cervical range of motion.

Cervicogenic headaches are one of the most rewarding conditions to treat with Physical Therapy because they respond so well when the underlying cervical dysfunction is accurately identified and addressed. Many people who have been managing cervicogenic headaches with medication for years find that a targeted Physical Therapy program addressing the upper cervical joints and surrounding musculature produces significant and lasting reduction in both headache frequency and intensity.

Treatment typically includes manual therapy directed at the upper cervical spine, targeted strengthening of the deep cervical flexor muscles, thoracic spine mobilization, and education about posture and movement habits that may be contributing to symptom recurrence.

Migraines with a Cervicogenic Component

Migraines are a complex neurological condition with multiple potential triggers and contributing factors, and they are not entirely within the scope of Physical Therapy to manage. Many presentations of migraine require medical management including medication and specialist care that goes beyond what Physical Therapy can provide.

That said, research increasingly recognizes that a subset of migraine presentations have a significant cervicogenic component, meaning that dysfunction in the cervical spine and surrounding musculature contributes meaningfully to migraine frequency, intensity, or duration.
